Leading the Conversation: Devoted Guardians Showcases Outcomes-Driven Innovation at HomeCare100

At this year’s HomeCare100 Summer Conference, a premier gathering of home care thought leaders, our very own Chief Operating Officer, Justin Tyler, took the stage during the “Provider Showcase: Leveling Up Your Core” session to present a message that hits at the heart of who we are at Devoted Guardians.
Representing our organization among a national audience of innovators, Justin shared the success of our Care Navigation Program—a forward-thinking model designed to reduce hospitalizations and readmissions by providing proactive, coordinated care to Medicaid members.
In an industry where outcomes are rarely discussed—especially among Medicaid providers—Devoted Guardians is changing that narrative. Our mission has always centered on compassionate, community-based care, but we believe compassion must be paired with measurable impact. Our Care Navigation approach demonstrates just that: through early identification of risk factors, improved communication with case managers, and seamless coordination across care teams, we’re improving lives while reducing costly and avoidable hospital stays.

Why This Matters
Too often, Medicaid populations are left out of the value-based care conversation.
