In partnership with Banner Health, our team of nurses, social workers, and certified navigators have helped over 500 members, providing 3,000+ touchpoints and nearly 300 referrals—all with one goal in mind: making sure our members get the care, resources, and support they need to live healthier, more independent lives.

Because of this program, hospitalizations have been reduced by 49%, and we’ve helped save over $5 million in healthcare costs—more importantly, we’ve helped bring peace of mind to families and improved quality of life for those we serve.
